The longtime partner of Sean "Diddy" Combs, she and the rap mogul share three children from their 13-year relationship.

TMZ claim law enforcement sources got a call to her home in Toluca Lake around noon on Thursday; the emergency dispatch audio reveals that the call came in as cardiac arrest.

A source connected to Kim tells us she'd been suffering flu-like symptoms, and possibly pneumonia, for several weeks, but it's still unclear what exactly caused her death. According to E Online, they had a son together named Christian (born in 1998) as well as twin daughters who are named D'Lila Star and Jessie James, born in 2006.

However, they broke up again - six months after the girls were born - with Porter saying it was her decision at the time.
